Certificate of Operation Process
If your Elevator Inspection Form (fig 1 below) is marked "PASS" in the lower left corner, fill out the *Application For Certificate Of Operation form (fig 2 below) and send both of these forms to the address at the top of the Application form. When the state receives these forms they will send you an invoice. Once this invoice is paid the State will send you a Certificate Of Operation which must be posted in the elevator. This same process applies for each elevator, escalator, or lift on your premises.
If your Elevator Inspection Form (fig 1) is marked "FAIL" in the lower left corner, contact your elevator maintenance company to repair the items marked/listed as deficient. Once the items are corrected or repaired, call Stuard & Associates at (765)352-1975 and schedule a "re-inspection". After your re-inspection is complete, follow the instructions for forms marked "PASS" above.
The above PASS/FAIL process for obtaining a Certificate Of Operation is the same for each Elevator, Escalator, or Lift.
(NOTE: Elevators,Escalators, and Platform Lifts each have their own respective inspection form.)
