Illinois - Application for Certificate of Operation - Annual
This is the form you need to renew your elevator(s) Certificate of Operation every year.
Completely fill out this form, one for each elevator you are renewing. Couple forms with your passing elevator inspection form(s) and send to the Fire Marshal’s address at the top of the form.
Illinois - Application for Certificate of Renewal - Triennial
NOTE: You can only renew with this triennial form if you meet all requirements at the top of the form.
This is the form you need to renew your elevator(s) Certificate of Operation if you renew every 3 years.
Completely fill out this form, one for each elevator you are renewing. Couple forms with your passing elevator inspection form(s) and send to the Fire Marshal’s address at the top of the form.
Take Elevator Out of Service
This form is given to you by our inspector after they complete the our of service inspection.
Send the completed form to the Fire Marshal’s address at the top of the form.
Change of Ownership
It’s pretty obvious what this form is used for. Wouldn’t it be great if all forms were named so well?
If you are the new owner of a building with an elevator, use this form to change owners for various reasons.
NOTE: You must fill and send this form to the address at the top of the form in order to officially change any information about your elevator.
The State will still accept this form via fax.
Registration of Conveyance
Completely fill out this form, one for each elevator you are registering. Your maintenance company can help you with some of the specific details of the elevator. All elevators in Illinois must be registered before they can have annual inspections.
Maintenance Authorization - 60 day extension
The state fire marshal does not allow elevator inspections or maintenance on expired or unregistered elevators without sending this form and getting their approval first.
Their turn around for this is quick and reliable.
